All MSPSP TEAMS:

 

 

All registration information will be collected July 8th and 9th at Carpathia Hall starting at 7:00.

 

Everything you need to register your teams is on www.mspsl.org., we  have envelopes from MSPSP, if anyone needs one please contact us

Please double check all of your information.  Each error is not only time consuming but costly.  Please remember that every registration form must be signed by MYSL as your affiliating league, if you are adding a late player that also needs to be signed by MYSL, you may fax it to 586 268 0849 or call us at 586 604 2458 and we will make arrangements to get it signed for you.  All dual roster forms must also be signed by us and they are also faxable. 

Even though MSPSP is no longer collecting proof of birth, it is MYSL responsibility to collect proof of birth and if asked provide MSPSP with requested proof.  No player will be registered without proof of birth. 

 

Registration fee’s are to be paid to MYSL; MSPSP does not accept club or personal checks. 

 

I will need a check for 375.00 per team; this will cover your registration fee to MSPSP and your affiliation fee for fall 2008.    MYSL will hire someone to pick up your pass cards and put them together for you.  If you would like this service please include a 25.00 check payable to MYSL and we will handle your pass card pick up for you.
